Psychological Assessment

At Malak Psychology, psychological assessments are conducted for adults and adolescents with the thoroughness, cultural humility, and individualized care that you deserve. Dr. Malak integrates multiple sources of information including clinical interviews, standardized testing, collateral information, and relevant records to ensure that her evaluations are comprehensive, defensible, and meaningful to your life and goals.

Assessments are not a one-size-fits-all process. Each evaluation is tailored to your specific referral question and may span multiple sessions to ensure accuracy and completeness. Dr. Malak does not usually see assessment clients for ongoing therapy to avoid dual role ethical concerns, though she will coordinate care with your existing treatment providers throughout the process. The exception is therapeutic evaluations, evaluations conducted specifically to aid treatment planning, which sometimes lead to patients continuing care with Dr. Malak and sometimes result in referrals elsewhere depending on their needs and goals.

Most evaluations are conducted virtually, with proctored testing administered securely online by either Dr. Malak, a practicum student, or another appropriate proctor. Patients in the Phoenix metropolitan area may conduct testing in person, though a computer will often still be used for data collection. You will likely want to bring lunch or snacks, and there is a refrigerator for you if needed.

For virtual assessments, some components may require materials to be mailed to you prior to your testing session. Please do not open or review any mailed materials until the psychologist or psychometrist instructs you to do so during your session. After testing is complete, please scan or photograph each page of your response booklet(s) and upload them to your patient portal or email them to:

Once sent, please delete the scans or photos from your email and device, as these are copyrighted materials that cannot be shared with or retained by unauthorized individuals. After scanning, please immediately enclose all response booklets and pencils without erasers in the pre-paid manila envelope provided and return them by mail as soon as possible. You will receive these instructions again verbally during your testing session.

Reports are typically completed within two weeks of your final testing session and include detailed findings, diagnostic impressions, and practical recommendations tailored to your needs.

Fees

$250 an hour plus materials which typically ranges from $2000-$4000 in total.

They can be cheaper when a student clinician is involved working under Dr. Malak’s supervision at $50 an hour for assessment time and $0 for their report writing typically ranging from $1000-$3000.
